While some PEDs are legally prescribed for medical purposes, their use outside of medical supervision is against the rules in most sports, including bodybuilding. The pediatric clinician must be aware of the use of performance-enhancing substances by pediatric patients; be prepared to identify risk factors, signs, and symptoms; ask screening questions; and offer anticipatory guidance related to their use. The reader should know that anabolic steroids are synthetic substances derived from testosterone and are also called anabolic-androgenic steroids (AASs) or just “steroids.” Steroid precursors also are used as performance-enhancing agents. Additionally, for outcomes where there were significant interactions between parent media practices and sex, parental education, or household income, we stratified results by the respective variable (Appendices E-G). Sex-stratified models showed that associations between certain media parenting practices (bedroom screen use, parental monitoring, and setting limits) and mature video games were stronger in males compared to females (Appendix E). Parental education-stratified models showed that associations between certain media parenting practices (e.g., setting limits) and mature video games and R-rated movies were stronger in households with higher than lower parental education (Appendix F).

A recent systematic review of AAS use found common reasons for seeking medical care were overall health concerns, blood test drug addiction monitoring, and prescription substances. Help with discontinuing AAS use was not a top priority.25 Clinicians should certainly discourage AAS use, but the initial interaction should serve to obtain a better understanding of why the patient is using AAS, what concerns they have, and why they are seeking care. Open-ended questions may reveal motivations of the patient, such as fertility or side effect avoidance. Identifying these factors creates opportunities to build rapport, minimize harm, and eventually progress to cessation. Of all AAS users, the statement indicates that 30% may develop AAS dependence.
Risk Factors and Side Effects

In some jurisdictions, the possession, distribution, and use of PEDs without a prescription may constitute criminal offenses, carrying penalties such as fines and imprisonment. Additionally, athletes found to be using PEDs may face disciplinary action from sports governing bodies, including suspension from competition and disqualification of results. Prior to entering the league, only the NFL requires pre-employment testing for all players, although in the MLB, all players are subjected to testing within five days of reporting for spring training. The NBA, NHL, NCAA, and all other professional leagues do not require pre-employment testing.

And I remember one of the team doctors came into my room and he was praising me about how well I had done, how much I’d pushed through the pain, and how impressed he was, but that I had to start taking care of my body and living a little bit more “professionally,” as he called it. He was wearing this fly fishing vest and he reached into one of the pockets and he pulled out this little red, egg-shaped capsule. It’s not doping, it’s for your health.” Yeah, it had testosterone in it.

I believe it would truly even the playing field because some countries (like Russia, which is currently banned from the Olympics until December 17, 2022 for their state-sponsored doping program) already are doping, while clean athletes are punished for being clean. There is a prisoners’ dilemma for athletes, with the dominant strategy being to cheat. This penalizes those who have the moral character or fortitude to not cheat, while rewarding others who did cheat. “But everyone else is doing it” might not be an argument a teenager is going to win with a parent, but in the case of professional athletes and PEDs, the argument is one that the sporting world should accept. If a player in any professional sport violates the league’s drug policy, they are immediately suspended, and the length of suspension (or ban) depends on how many times the player has violated the drug policy. The NFL and MLB also test for “drugs of abuse” (recreational drugs, i.e., marijuana, cocaine, etc) and the NBA, similarly, tests for cocaine, opiates, PCP, LSD and marijuana.
- Performance enhancing drugs (PEDs) have long been a topic of discussion within the bodybuilding community, stirring various opinions among athletes, fitness enthusiasts, and the public.
- Parents, coaches, and the players themselves are constantly pushing themselves to perform.
- In 2001, Metzl et al (23) questioned 1,103 adolescents and found creatine use in 5.6% of them.
- We believe this approach further propagates distrust of physicians, encourages continued reliance on other men using AAS for guidance, and reduces the likelihood of eventual cessation.
- Thomas O’Connor owns and operates a private practice internal medicine clinic based out of Essex, CT, and has written a book on the adverse effects of AAS misuse.
Although there has been considerable media speculation that erythropoietin could have been implicated in the deaths of as many as 18 European professional bicycle racers between 1987 and 1991, there is no forensic documentation from verifiable sources substantiating this claim (388, 389). Meta-analyses ped usage of randomized trials in patients with cancer and in those with end-stage renal disease have revealed an increased risk of mortality, thromboembolic events, cardiovascular events (including myocardial infarction and stroke), and hypertension. Some of the adverse effects seen in patients who use AASs may include infertility, gynecomastia, sexual dysfunction, hair loss, acne, muscular appearance, and testicular atrophy. Some indicators that might suggest AAS use are increased hemoglobin and hematocrit; suppressed LH, FSH, and testosterone levels; low high-density lipoprotein cholesterol, and low sperm density. Mass spectrometry-based tests (available in many commercial laboratories) can detect AASs in urine. Testosterone abuse is more difficult to detect, but high testosterone, in association with suppressed LH and FSH levels, should raise suspicion of testosterone abuse.
- Despite its enormous promise, the progress in the gene therapy field has lagged substantially behind the early expectations because of technological and safety issues.
- By infusing insulin along with stable isotopes of glucose and amino acids into human muscle (quadriceps), Biolo and coworkers (386) were able to demonstrate an approximately 70% increase in the fractional synthetic rate of muscle protein.
- A T/E ratio of more than 4 can confirm testosterone abuse, although it is rarely necessary to check testosterone levels in the clinical setting.
- Performance Enhancing Drugs, commonly referred to as PEDs, are substances or drugs used by athletes to improve their physical attributes artificially.
